How do I...

How do I submit a death claim?
What's needed: To expedite your claim, call or send your request to obtain the Claim Forms, including the following information:
  • name of the deceased, and the dates of birth and death
  • policy number(s)
  • cause of death (natural or accidental)
  • funeral home name, address and phone number; and
  • beneficiary's name, address and phone number


A Sun Life Financial representative will return your call within 24 hours, or if you wish, mail you the information within 5 days.The designated beneficiary will need to provide proof of death (i.e., certified copy of the death certificate or a notarized attending physician's statement) and a completed claim form. Sun Life Assurance Company of Canada (U.S.) pays death benefit proceeds directly to the named beneficiary via an interest bearing account, the Sun Financial Benefit Account. For beneficiaries' convenience, the account offers checkwriting privileges, allowing them to write a check for the all or part of the funds as needed, leaving the account open as long as they desire. Beneficiaries will receive a monthly statement showing all checks written and interest credited to the account. (Due to various state regulations, there are some states where Sun Life Financial does not offer this payout option automatically.)

How do I submit a waiver of monthly deductions claim?
What's needed: To expedite your claim, call or send your request to obtain the Claim Forms, including the following information:
  • name of the insured and policy owner (if different)
  • policy number(s)
  • the date the disability commenced and the cause of disability; and
  • address confirmation for mailing the necessary forms


A Sun Life Financial representative will return your call within 24 hours, or if you wish, mail you the information within 5 days.

The claimant will need to complete the Disability Claim Package and an authorization to obtain additional medical information if necessary, provide a Physician's Statement completed by the treating physician, and provide a statement from his or her employer.

Upon receipt of the necessary requirements, the company will inform you of the decision to waive the premium within 5 days.

To be eligible, your policy must have a Waiver of Premium benefit. Disability must be for a duration of at least six months, in accordance with the policy's waiting period provision. The company must be notified while (a) the disability is ongoing during the insured's lifetime and (b) as long as the policy is in force.

What's needed: To expedite your claim, call or send your request to obtain the Claim Forms, including the following information:

My policy recently lapsed. How can I reinstate it?
What's needed: If you wish to reinstate your Sun Life policy, call or send your request, including the policy number(s). Sun Life Assurance Company of Canada (U.S.) will (a) provide you with a quote of back premiums due and overdue premium interest, and (b) determine if evidence of insurability is required. The company allows a five year grace period for reinstatements. After that period the reinstatement offer expires and the coverage will remain terminated.
